  9. When I claim reimbursement, in the claim form, for policy number, should I enter my card number or the group policy number? And what is the certificate number and TPA id number?

  10. Where can I find my tpa id in card?

  11. I have joined the IBA Medical Scheme first time on 19.09.2016 and basing on my application dated 19.09.2016,
    they have debited Rs 5645/- pa on 29.09.2016 and the premium is for 2016 – 2017. And later in the First week of October, the UIICO has increased the premium amount to Rs 12,020/- for clerks (non-domiciliary)
    for the year 2016 -17 and so instead of debiting differential amount, they have once again debited the full amount of Rs 12020/-.

    So please inform me whom I have to approach for the refund of the Rs 5645/-.
    I also request you to inform me when I should get the ID card though almost 2 months lapsed since my payment.
    Please take up the matter with higher authorities as we may loose some of our illness admissions in to the hospitals due to non receipt of ID Cards.
    For non refund of the excess premium and non-issue of e-health card, whom shall I give complaint.
    Please inform me.

    My reply: The bill is retained by Vidal hence it is not possible to submit certificate from doctor/bank officer, unless the bill is returned for doing needful.
    What should be filled in the enclosed format?

    South Indian bank ltd card no Koc Ui 10585 002 0001190 A. The card issued affixed the photograph of my husband Poulose instead of myself Leelamma varkey. This was reported in your Ernakulam office and they forwarded the matter to your card issue dept. Nothing heard after that. What is the position? Please issue the card with proper photograph without delay. It is pending for the last one month.

The Branch where I get Pension, Mahanagar Boys school , Lucknow , Branch code No 8483 also did not send it to me in spite of several visits there. I request you to kindly arrange to send it at the address mentioned in the insurance application.

  35. I am not sure whether I can get reimursement for medicines and periodical consultations for chronic ailments such as Astma and hypertension treatments not requiring hospitalisation.
